Application to Change Status
Students who are non-degree and who
want to continue their enrollment in a degree
or certificate program should complete the Application
to Change Status Form. Non-degree students are not eligible
for financial aid.

Students who wish to change from one degree or certificate program to another and/or change their name and/or address should use the Change Request Form.

Enrollment Verification
Students seeking enrollment verification can print
out their own Enrollment Verification Certificate via
the Internet.
- Logon to http://my.commnet.edu
- Login using your NetID
- Go to the Student Tab
- Click on the “Student Self Service” Channel
- Click on “Student Records”
- Click on “Request for Enrollment Verification”
- Click on “NSC Self-Service” (Direct connect to National Student Clearinghouse)
- Click on “Obtain an Enrollment Certificate”
- Print your Certificate!
Enrollment verification is generally accessible beginning the third week of each semester.
Grade Reports
Semester grades are viewable and printable via the web at www.my.commnet.edu approximately one week after the exam period.
- Logon to http://my.commnet.edu
- Login using your NetID
- Go to the Student Tab
- Click on the “Student Self Service” Channel
- Click on “Student Records”
- Click on “Final Grades”, select term then submit OR
- Click on “Academic Transcript”, select level (CCC Credit), then submit
- View/Print your final grades/transcript
- Exit from the website
Students may request to have their grades mailed to their home address by completing a Grade Report Request Form.

Graduation
Students who anticipate completing the requirements
for an Associate Degree and/or a Certificate, must submit
a Graduation Application by February 15th
of the year they plan to graduate, whether or not they
will be attending the graduation ceremony.

Readmission
Readmit students are those who have previously been
accepted and who have attended Mattatuck Community College,
Waterbury State Technical College or Naugatuck Valley
Community- Technical College, but who have been away
from the College for two years or more.

Transcript Request Form
Students may request an unofficial copy of their transcript
for their own use at no charge. Click and print the
Transcript Request Form, and supply the information
for your request. Mail or fax your request to the attention
of the Records Office @ 750 Chase Parkway, Waterbury,
CT. The fax number is 203-575-8085.
